We have specialist Social Prescribers in practice who can see patients for non-clinical advice and specialist support for anything that is impacting on their health and wellbeing. Examples of support offered includes:
- Befriending services
- Bereavement help / referral to specialist services
- Education support
- Employment support
- Financial support including assisting patients to apply for welfare benefits
- Housing support
- Isolation
- Training support
- Volunteering opportunities
Our Social Prescribers work alongside our GPs to provide specialist and focused support. If you need onward referral to specialist advice or help, our Social Prescribers can arrange this for you.
