New Patient Registration

New Patient Registration

Registration is quick, simple and open to all residents in our practice boundary. Please complete and submit the online form below to register.

Please note that incomplete forms will be returned to you – delaying your registration. Please ensure all sections of this are read and completed fully.

Find your NHS number
You do not need to know your NHS number to use NHS services, but it can be useful to have it.

New Babies

New parents must register their baby as soon as possible after birth with a GP practice – preferably with the same practice as the parents.

Dependence Forming Medicines Practice Policy And New Patients

Opioids (morphine-like medicines), pregabalin, gabapentin, benzodiazepines, and sedatives are dependency forming medicines. Any new patients registering with the practice must inform the practice staff if they are prescribed any of the below medications.
