Communication Preferences
Patients will be asked about their communication preferences when they register and you can update these with us at any time by using this form.
We usually send invitations, updates and other information to patients by text message or letter where the patient has no mobile number listed in their record. Some patients also provide their email address for communication. If you receive an invitation to book an appointment please do so either using the booking link on the text, using the NHS app or by telephone 01254 369123.
Power of Attorney / Next of Kin
Please inform us if you have a ‘Power of Attorney' in place for health or if you would like us to communicate with your next of kin or someone else on your behalf so we can add this to your preferences.
Summary Care Record
Patients are encouraged to allow information held by their GP to be uploaded onto the universal health Summary Care Record. This allows other NHS organisations to see important information about you and your health which may be important in an urgent or emergency situation. Examples include allergies, conditions you have, medication, next of kin etc. We recommend all patients remain opted in to the Summary care Record. Please use this form to tell us if you wish to opt out of a Summary Care Record.
Additional Needs / Reasonable Adjustments
For vulnerable patients with additional needs, please complete this form so that we can make adjustments according to your needs.
