Patient Responsibilities

- Order repeat prescriptions responsibly and in advance (allowing at least 48 hours) using the NHS App, Drop Off at the surgery, Post (enclosing a SAE), Practice Website.
- Use urgent and emergency services appropriately.
- Be open and honest with healthcare professionals about your health and lifestyle.
- Follow agreed treatment plans and attend follow-up appointments, as necessary.
- Notify us of any changes to your personal details (address, phone number, next of kin, etc.).
- Respect practice policies and procedures that ensure fair access for all patients.
