Practice Charter

At Oswald Medical Centre, we provide high-quality, patient-centred healthcare in a safe, respectful, and supportive environment. We aim to work in partnership with our patients to promote health, wellbeing, and continuity of care.

We will:

  • Provide the highest standard of medical care within the resources available to us.
  • Treat all patients with dignity, respect, and courtesy, regardless of age, gender, disability, race, religion, or sexual orientation.
  • Always ensure patient confidentiality in line with GDPR and NHS guidance.
  • Offer same-day urgent appointments when medically appropriate and routine appointments within a reasonable time frame.
  • Provide clear explanations about your health, treatment options, and care plans, enabling you to make informed decisions.
  • Refer you to specialist services or hospital care when clinically appropriate.
  • Prescribe medicines safely and responsibly, in line with national guidance.
  • Keep our practice environment safe, clean, and accessible.
  • Welcome feedback, comments, and complaints to help us improve our services.

Patient Rights

As a patient registered at this practice, you have the right to:

  • Access NHS services without discrimination.
  • Be treated with courtesy, dignity, and respect by all members of the practice team.
  • Confidentiality and privacy always, including access to your medical records in line with the law.
  • Clear, understandable information about your condition, treatment, and care options.
  • Be involved in decisions about your healthcare and to give or withhold consent.
  • Request a second opinion or referral when appropriate.
  • Receive urgent care when clinically necessary.
  • Complain, comment, or provide feedback about the service you receive, without prejudice to your care.

Patient Responsibilities

To help us provide the best possible care, we ask that you:

  • Treat staff, clinicians, and other patients with courtesy and respect.
  • Keep appointments or cancel them in a suitable time to avoid wasted appointments.
  • Order repeat prescriptions responsibly and in advance (allowing at least 48 hours) using the NHS App, Drop Off at the surgery, Post (enclosing a SAE), Practice Website.
  • Use urgent and emergency services appropriately.
  • Be open and honest with healthcare professionals about your health and lifestyle.
  • Follow agreed treatment plans and attend follow-up appointments, as necessary.
  • Notify us of any changes to your personal details (address, phone number, next of kin, etc.).
  • Respect practice policies and procedures that ensure fair access for all patients.
